YAMA Foundation collects your personal information through your submission of this Service Request Form.
1. How we use your personal information
For us to process your service requests and provide information and services, we will require you to provide your personal information to us. YAMA Foundation will use your personal information for requests registration, for provision of services, and for marketing and promotional and membership purposes. Please note that failure to provide sufficient personal information as requested in the mandatory fields on this Service Request Form will render us unable to provide your requested services.
2. How we share and disclose your personal information
The personal information held by YAMA Foundation will be kept confidential, but we may provide such information to our contractors or third-party service providers who process applications and orders and provide mailing services, marketing support, and research activities on our behalf. To enable their execution of the above services, we will need to provide them with access to personal information on a minimum need-to-know basis. However, we will impose requirements of security and confidentiality on their data handling. They will not be permitted to use the personal information for purposes other than providing the required services set out above.
Unless we have informed you beforehand and have obtained your authorization or are obliged to do so by applicable laws and regulations, we will not under any circumstances publicly disclose your personal information.
3. Direct marketing
Your name, mobile phone number and email address collected by us will be used for providing you with information for marketing and promotional services. We cannot use your personal information unless we have received your consent or an indication of no objection. You can unsubscribe from all marketing communications. Every time you receive a mail or an e-mail from us, you will be provided with the choice to opt out of future mails or e-mails by following the instructions provided therein. You may also opt out by contacting us if you no longer wish any of the personal information provided to be used in any of the above-described direct marketing purposes.
4. Security
We employ necessary technical, organizational and physical safeguards designed to protect the personal information we collect. However, security risk is inherent in all internet and information technologies, and we cannot guarantee the security of your personal information.
5. Your rights
According to the Hong Kong Personal Data (Privacy) Ordinance, you have the right: to check whether we hold personal information about you and to access such data; to require us to correct inaccurate data; and to ascertain our policies and practices in relation to personal information and the kind of personal information held by us.
