我們的服務很簡單：為香港最脆弱的社群提供服務，給有特殊需要，殘疾，慢性病或貧困的社群提供免費或有補貼的外展計劃。
我們的服務包括三大範疇 :
Our service is simple: We bring free or subsidised outreach programmes, serving Hong Kong’s most vulnerable communities, including those with special needs, disabilities, chronic illness or disadvantage of any kind.
We serve five core areas:
有特殊需要或患罕見疾病兒童
Children with Special Needs, Rare Diseases, Illness or Living in Poverty
主要服務對象為罕見疾病兒童，以及有特殊需要或殘疾的兒童。此外，我們亦會幫助貧困兒童，滿足其個別的需求。
The core of our service is dedicated to children with special needs, those living with cancer or rare genetic diseases, or living in difficult circumstances.
殘疾或有慢性疾病的成年人
Adults with Disabilities, Chronic Illness or Vulnerability
在香港，大約10％的人口有特殊需要、殘疾或慢性疾病。此外，15％的人生活在貧困中。我們為護理院提供服務，並與慈善機構合作。
Approximately 10 percent of Hong Kong's population lives with a special need, disability or chronic illness. Additionally, 15 percent of our population lives in poverty. We reach out to adults in residential care homes and parter with other charities.

YAMA Foundation 服務本港廣泛的社群，包括支援專幫助有不同需要人士、家庭和照顧者之非政府組織所舉辦的活動。
YAMA Foundation helps support the wider community, including events that support other NGOs who serve Hong Kong's differently-abled communities, families and carers.
